Frodo's scar is slightly more straightforward and less mysterious. It was given to him by the Witch-king of Angmar, the leader of the Nazgûl, with a Morgul blade at a disused watchtower on Weathertop. He is then taken to Rivendell, where he is healed by Elrond.
As it wasn't actually Sauron who stabbed Frodo, it cannot be said that it was the "ultimate" villain who gave him his scar.
