In the theatrical release, we initially see four orcs descending the stairs. After we see Sam's shadow and hear him growling as he climbs the stairs, we flash back to the Uruks--and now there are only three. One of them has run up the stairs, but if you look closely, it's not the same orc that confronts Frodo later. The mystery is solved in the Extended Version, where we see the fourth Uruk slinking away with the mithril shirt! That makes four Uruks on the stairs--and the leader that intends to kill Frodo makes five!
